Why are these here? As a licensed telehealth medical practice, we are required to clearly communicate the scope, limitations, and conditions of our services. These disclosures protect both you as a patient and our practice. We encourage you to read them — transparency is part of how we operate.

Same-day cancellations or missed appointments (no-show) The full appointment fee will be retained

Payment is collected at the time of scheduling. All cancellations must be submitted in writing via the patient portal or by direct message to the practice. Cancellation timing is calculated from the scheduled appointment start time.

The no-show/same-day cancellation fee applies to all missed appointments, including monthly member visits, and will be charged to the card on file. Repeated no-shows may result in discharge from the practice.

This policy is acknowledged and agreed to at the time of booking. For questions, contact us directly before your scheduled appointment.

Not all patients will be eligible for GLP-1 medications or other treatments offered through this practice. Eligibility is determined solely at the clinical discretion of the provider following a comprehensive evaluation. Scheduling an appointment does not guarantee treatment.

Telehealth services are available to patients who are physically located in New York State at the time of their appointment. It is the patient's responsibility to ensure they meet this requirement. Appointments conducted outside of New York State may be canceled or considered invalid.

This practice does not provide emergency medical services. If you are experiencing a medical emergency, please call 911 or go to your nearest emergency room immediately. Telehealth services are not a substitute for emergency care.
